windshield wiper syndrome) is one of the most common causes of lateral knee pain, particularly among long-distance runners, cyclists, and hikers. It is insidious, often beginning as a diffuse discomfort before developing into sharp, debilitating pain. It is sometimes misunderstood, confused with other knee pathologies, and can be difficult to treat without a comprehensive, individualized approach.<\/p>\n\n\n\n<p>This syndrome highlights a frequent paradox in musculoskeletal pathology:&nbsp;<strong>it is not the most obvious structure that suffers, but the one that compensates<\/strong>&nbsp;, that absorbs imbalances in posture, gestures or terrain. This is where osteopathy offers a valuable reading, by placing this pain in a&nbsp;<strong>broader biomechanical, functional and even emotional context<\/strong>&nbsp;.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"What_is_iliotibial_band_syndrome\"><\/span><strong>What is iliotibial band syndrome?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>The iliotibial band, also called the iliotibial tract or fascia lata, is a fibrous structure stretched between the iliac crest (at the pelvis) and the tibia, via the external lateral condyle. It is reinforced by the muscular insertions of the tensor fascia latae (TFL) and the gluteus maximus, two key muscles in locomotion and postural control.<\/p>\n\n\n\n<p>Its main function is to&nbsp;<strong>stabilize the knee laterally when walking, running or jumping<\/strong>&nbsp;. It acts as a strap that limits excessive lateral movements, especially during the stance phase.<\/p>\n\n\n\n<p>In the syndrome, the band repeatedly rubs on the lateral condyle of the femur during knee flexion\/extension, causing local soft tissue inflammation and pinpoint pain, often described as&nbsp;<strong>burning or stinging on the outer side of the knee<\/strong>&nbsp;, aggravated by exertion.<\/p>\n\n\n\n<p>Unlike classic tendinitis, this is&nbsp;<strong>a friction conflict rather than simple muscular overuse<\/strong>&nbsp;. But as is often the case with functional pathologies, this friction is the&nbsp;<strong>result of a more global imbalance<\/strong>&nbsp;: hip dysfunction, poor femoral rotation, collapse of the plantar arch, weakness of the pelvic stabilizers, etc.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Why_does_it_particularly_affect_athletes\"><\/span><strong>Why does it particularly affect athletes?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>Iliotibial band syndrome is emblematic of what are called&nbsp;<strong>overuse syndromes<\/strong>&nbsp;, common among regular athletes or overly enthusiastic beginners. The repetition of movements \u2013 typical of running \u2013 exposes this anatomical structure to cumulative mechanical stress, particularly when running downhill, where the gluteal muscles and the TFL work more eccentrically to stabilize the knee.<\/p>\n\n\n\n<p>Several factors promote this syndrome in athletes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Too rapid an increase in training intensity or volume<\/strong>\u00a0, without progressive adaptation.<\/li>\n\n\n\n<li><strong>Inclined or uneven terrain<\/strong>\u00a0, such as slopes or inclines, which accentuate pelvic tilt and dynamic knee valgus.<\/li>\n\n\n\n<li><strong>Poor biomechanical alignment<\/strong>\u00a0, often ignored: foot collapse, internal rotation of the femur, pelvic asymmetry.<\/li>\n\n\n\n<li><strong>Improper or worn footwear<\/strong>\u00a0, altering the way forces are absorbed by the lower kinetic chain.<\/li>\n\n\n\n<li><strong>Muscle fatigue<\/strong>\u00a0: Deep stabilizing muscles (especially the gluteus medius) can lose effectiveness over the kilometers, leaving the band to compensate excessively.<\/li>\n<\/ul>\n\n\n\n<p>This syndrome is therefore often&nbsp;<strong>the symptom of a series of poorly integrated compensations<\/strong>&nbsp;in the sporting movement.
