In most cases, it results from a&nbsp;<strong>large lumbar disc herniation<\/strong>&nbsp;, often located between L4 and S1, which protrudes into the spinal canal and compresses the nerve roots. However, other causes exist: tumors, infections (epidural abscess), severe spinal stenosis, vertebral trauma, or post-surgical complications. Whatever the origin, the common denominator remains the acute compression of these fragile nerves within an anatomically confined space.<\/p>\n\n\n\n<p>This compression triggers a cascade of pathophysiological phenomena: inflammation, ischemia, and axonal degeneration. Within hours, nerve signals are interrupted, muscles become paralyzed, and sphincters disconnect. This is why&nbsp;<strong>cauda equina syndrome is an absolute surgical emergency<\/strong>&nbsp;. The later the intervention, the more irreversible the damage. The clinician must know how to ask the right questions:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>How long has the pain been present?<\/li>\n\n\n\n<li>Is it associated with urinary or digestive problems?<\/li>\n\n\n\n<li>Does the patient experience numbness in the pelvic region?<\/li>\n\n\n\n<li>Can he walk or stand?<\/li>\n<\/ul>\n\n\n\n<p>Once suspicion is established, the diagnosis is confirmed by&nbsp;<strong>medical imaging<\/strong>&nbsp;, particularly&nbsp;<strong>lumbar MRI<\/strong>&nbsp;, which allows visualization of the nerve compression and determination of its cause. This examination, performed urgently, guides the surgical decision.<\/p>\n\n\n\n<p>MRI often reveals an&nbsp;<strong>extruded herniated disc<\/strong>&nbsp;invading the spinal canal, an&nbsp;<strong>epidural abscess<\/strong>&nbsp;, or a&nbsp;<strong>tumor<\/strong>&nbsp;. Sometimes, the cause is&nbsp;<strong>severe degenerative stenosis<\/strong>&nbsp;in elderly patients. Imaging also allows assessment of the severity of the compression and estimation of the likelihood of recovery based on the duration of symptoms.<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-vivid-cyan-blue-color has-text-color has-link-color wp-elements-d2d1acb31834a871073f5be2fe9abd27\"><span class=\"ez-toc-section\" id=\"Time_a_decisive_factor_in_avoiding_the_irreversible\"><\/span><strong>Time: a decisive factor in avoiding the irreversible<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>In cauda equina syndrome,&nbsp;<strong>time is the primary enemy<\/strong>&nbsp;. Every hour of compression prolongs nerve damage and reduces the chances of reversibility. After 48 hours, the chances of a full recovery decrease drastically.<\/p>\n\n\n\n<p>Even after successful surgery,&nbsp;<strong>lasting effects can persist<\/strong>&nbsp;: muscle weakness, incontinence, and chronic neuropathic pain. This is why early recognition of warning signs is key to prognosis.
