This pain may be aggravated by walking, climbing stairs, prolonged standing, or sleeping on the affected side. It can also occur when the pelvis lacks stability while bearing weight on one leg.<\/p>\n\n\n\n<p>Gluteus medius syndrome is important to recognize because it can be mistaken for sciatica, although its path is often more lateral. The gluteus medius plays a major role in stabilizing the pelvis. The term is often used whenever pain originates in the lower back, crosses the buttock, or travels down the thigh. However, several musculoskeletal disorders can produce very similar pain without necessarily involving lumbar nerve compression.<\/p>\n\n\n\n<p>Piriformis syndrome, deep gluteal syndrome, gluteus medius syndrome, sacroiliac joint dysfunction, proximal hamstring tendinopathy, coccydynia, greater trochanteric pain syndrome, and referred mechanical low back pain can all mimic sciatica. The location, aggravating factors, the presence or absence of neurological signs, and the response to movement help guide the evaluation.<\/p>\n\n\n\n<p>The key is not to treat only the path of the pain, but to understand its origin. True sciatica requires particular attention to neurological signs. False sciatica, on the other hand, requires an analysis of the muscles, tendons, pelvis, hip, posture, gait, and postural compensations.<\/p>\n\n\n\n<p>Osteopathy can play a useful role in this overall assessment, helping to improve mobility, reduce tension, support the return to movement, and guide the patient toward more suitable exercises. However, a cautious approach remains essential: weakness, significant numbness, urinary or bowel problems, fever, unusual nighttime pain, or pain following trauma should always prompt a prompt medical evaluation.<\/p>\n\n\n\n<p>Better distinguishing sciatica from its imitators helps avoid treatment errors, inappropriate stretches, and unnecessary anxiety.
